    Service Coordinator Lead

    Key Responsibilities:

    • Supervise, coach, and support Direct Support Professionals (DSPs)
    • Oversee ISPs, quarterly reviews, and required documentation
    • Ensure compliance with DBHDS licensing and Medicaid Waiver standards
    • Lead quality improvement, risk management, and service coordination efforts
    • Collaborate on daily staffing and transportation problem-solving
    • Coordinate communication with families and caregivers
    • May provide direct support services on a limited, as-needed basis to ensure continuity of care during staffing emergencies.
